What removes a row
A row leaves the register when the surface behind it can no longer be opened at the address recorded, or when a re-read on a later date changes what the surface carries. A row is never removed to improve an impression, and no row is added for a page nobody opened. A source the fetch log records as failing to load is left out entirely, because a failed fetch is a fact about tooling and not about whatever sits behind it.
