Three findings, named apart
A register that reads pages produces three findings that look alike from a distance and are not. A name can be absent from a page that was opened. A read date can be blank, because nobody opened the page. And a file can be published and linked and still never read here. This page keeps the three apart, because collapsing them is the single error most likely to be checked.
Absent
Absent means the page was opened on the date stamped beside it and this name was not on it. That is the whole finding. It does not mean a document does not exist, was refused, or was hidden. Converting an absence on one surface into a claim about the world is the error this rule exists to stop.

Blank
A blank read date means nobody opened that page. In the surface audit, one row carries no date, and the missing date is the finding on that row rather than a gap in the record. An unopened page cannot report an absence, because reporting an absence requires opening the page first.
Published, linked, and unread
A file can be published, linked, and never opened here, and that is a third state again. The certificate index publishes and links its panels, and across this whole programme only three panels and two report images came down, none of them this compound's.
